Textbook Scholarships for Survivors
SurvivorVision's Textbook Reimbursement Scholarship Program: Helping Survivors Make the Grade!
SurvivorVision is very excited to announce our Textbook Reimbursement Scholarship Program. This initiative is specifically geared towards pediatric cancer patient/survivors. We hope to empower cancer patients/survivors to live the best life possible after cancer. This scholarship will help young cancer survivors to obtain a college education in order to attain their personal, professional and educational goals.
Eligibility: This program is available to cancer patients/survivors treated or living in Illinois and Indiana. Students must be enrolled in a post-secondary educational institution (i.e. college or associate, trade school or graduate school). Students must have been diagnosed before 18 years of age.
Application Timeline: Submissions for the Fall program are now closed. Applicants will be notified beginning December 1, 2011 of their acceptance into the Textbook Reimbursement Program.
How to Apply: All Applicants must enclose the following: Please submit one copy of each and keep one copy for your records.
- Letter from your physician verifying your diagnosis
- Completed Textbook Reimbursement Scholarship Program Application

Scholarship Terms and Reimbursement Schedule:
- $1,000/year maximum with a lifetime maximum of $5,000 over 5 years.
- Applicants need to reapply annually to be eligible for each year's reimbursement payouts.
- Approved Education Textbooks will be paid out as follows:
- First payout - January 15, 2012- Required documents due December 15, 2011

- Copy of your class schedule (course schedule/registration must have official confirmation of your name as registered in courses.

- A list of books required for each course is necessary as proof of need
- Include and itemize the application shipping/handling fees (standard ground/UPS only) and pertinent taxes on your Reimbursement Form.
Receipts for items/supplies other than texbooks are subject to review by SurvivorVision's Board.
Please note: All receipts need to be received by the due date in order to receive payout for that payment cycle. We strongly recommend that you keep a copy of all your receipts and reimbursement forms. SurvivorVision is not responsible for lost mail or missed deadlines.
